Health insurance 2027: the cheapest offer costs 600 francs a year less than the median offer

Health insurers had to announce their 2027 premiums by 31 October, and a cancellation of basic insurance must reach them by 30 November 2026, the Federal Office of Public Health (FOPH) points out. For the same cover, the cheapest offer in the region costs about 600 francs a year less than the median offer, at the median of municipalities, according to Civiora’s calculation. Basic insurance benefits are the same with every insurer.

The deadlines before the end of the year

Cancellation of basic insurance must reach the current insurer by Monday 30 November 2026 at the latest. The FOPH advises sending it in writing by 15 November, by registered letter or “A-Post Plus”; this year, 15 November falls on a Sunday. At the same time, you must join the new insurer, usually with its membership form. The old cover only ends once the new insurer has confirmed continuous cover.

The same deadlines apply to other changes. A lower deductible must be requested in writing by 30 November, a higher one by 31 December. Leaving an alternative model, such as a family doctor or telemedicine plan, must also be notified by 30 November; switching from the standard model to an alternative one is possible at any time of year.

Same cover, different premiums

Compulsory health insurance benefits are the same with every insurer: switching does not change basic cover, according to the FOPH guide. Insurers must accept any new policyholder without reservation or waiting period, whatever their age and state of health, and may not require a health questionnaire. The only exception: anyone who still owes premiums or cost-sharing that received a reminder by 30 November and has not paid them by 31 December cannot leave their insurer.

What Civiora’s calculation shows

Civiora compared all 2027 offers in the FOPH’s official file for an adult aged 26 and over, with a 300-franc deductible, accident cover and free choice of doctor. Depending on the premium region, 21 to 28 insurers offer this model. At the median of the 2’102 municipalities in the 42 regions, the most expensive offer costs 1’580 francs a year more than the cheapest, and the median offer about 600 francs more.

The gap between the cheapest and the most expensive insurer varies by region: from 940 francs a year in the premium region of Sion (VS) to 3’170 francs in the canton of Uri. Premiums are set by region and canton: within one region, every municipality sees the same offers.

The insurance model, a second lever

Alternative models (medical practice, phone or app, pharmacy) restrict the choice of providers in exchange for a lower premium. For the same person, the cheapest alternative offer in the region costs about 850 francs a year less again than the cheapest standard offer, at the median of municipalities, according to Civiora’s calculation. The FOPH recommends first choosing the model that suits your needs, then comparing insurers with its priminfo calculator, which also works out the possible saving.

Supplementary insurance: different rules

Cancelling basic insurance does not oblige you to cancel supplementary insurance with the same insurer, and the insurer may not cancel it on its own initiative. Cancellation periods for supplementary insurance are generally different and are set out in its general terms. On priminfo, the FOPH provides model letters for cancellation and membership. Civiora recommends no insurer: the calculation is based on published premiums, not on quality of service.

In short

  • 600 CHF: less per year for the cheapest offer than for the median offer, at the median of municipalities
  • 1’580 CHF: gap per year between the most expensive and the cheapest offer in the region, at the median of municipalities
  • 30.11.2026: date by which the cancellation must have reached the current insurer (FOPH)
